Britney has a debilitating enough mental illness that she can’t even control her own life, nor can she be unsupervised for any period of time. It’s way deeper than just being a shit show.

I think she may be drinking more than she is letting on. This would account for the bloated face, rough/emotional mornings, gray look to her skin, and lack of weight loss. Always having to pull herself together, find peace, etc.
I don't want to speculate that she is drinking excessively or make assumptions about addiction. But many people self medicate with alcohol.

As someone who is following keto strictly to lose 15 pounds worth of COVID lockdown drinking, I’d say it’s a good guess as to why the over exercising isn’t showing the results it should.
